This can include anxiety, depression, bipolar disorder, Post Traumatic Stress Disorder (PTSD), and adjustment disorder.  A mental health assessment gives your counselor  a complete picture of your emotional state.  The therapist will ask you why you are seeking counseling, how the current problem is impacting your life, your goals of therapy, and your history.  Based off the mental health assessment  a  treatment plan is developed  that helps your therapists and you define and monitor your mental health treatment.
